Need input on NEHA medical form

Why is there hospital information? If I have a accident, I want to go to the nearest hospital that can take care of my injuries.

Should there be a records release pre-signed with this to get around HIPA? Do we have HIPA implications with maintaining even such minor medical records?

Hospital is include so that the local hospital has a place to call in addition to the doctor's office (he is playing golf and not in the office). More options.

I don't have an answer regarding HIPA.
