Making a fillable form (final product should be in Adobe) that will be available on the NEHA website for download.

I made some changes that I felt made sense; looking for your input.

I removed the address of the medical contact person. Was there a need for it?

Modified signature section to include "first responders".

Regarding the checkbox section... what should be added or removed (I don't understand the importance of some of the items).

Looks OK to me.

Not sure about the check boxes but I wouldn't remove any unless Speedway says it's OK. There may be some important reason why they want to know about false teeth even if I have no clue why that's important.

Great minds think alike (and then there's us) - had started one of these during Philo, somewhat to 'clean-up' the quality, but mainly because we were running very low (like one or two Friday night) - mine was a word.doc, will see if I can find it...

+1 on the Speedway approval thing - a lot of these forms, when used for years, have the advantage of speed when seeking information - knowing that the info you need is in that spot...

Dentures/partials can become a choking hazard during CPR or other breathing assistance techniques...

I just sent it to Speedway for their input.

Yes, it should. I actually meant to include it at the top. Thanks for the reminder.

Speedway reviewed form and said all was good; they requested "Medical History" section.

See the links above for both PDF form and Word form. I did not want to spend the time formatting the fields (i.e. dates, phone numbers, etc) to avoid issues when trying to fill them out (i.e. Hey, I'm from Canada, how do I put my phone number and ZIP.... opps, I don't have a ZIP... code into the fields?)

Cool I'll have to fill one out and bring it the first event.

I will figure out where to post this on the website (might just include it within each event page). I'm thinking that we should require everyone to fill out a new form this year to get a fresh start so we can shred the current forms. Then, we can work off the dates of each form to make sure the person is still active after (for example) 5 years... if not, off to the shredder it goes. No need to hold onto this stuff forever.

And as a bonus the forms filled out via these new forms will probably be legible and more accurate. If I'm filling it out at home I can go look up my doctor's information at the hill forget it.
